What do we mean by Personalised care?
Personalised care helps people living with cancer to take an active and empowered role in the way their care is planned and delivered, with support and care tailored around the things that matter most to them.

Personalised care was a central theme in the NHS Long Term Plan. It outlined clear ambitions for personalised care for all users of health services, including how we support people with cancer.
The NHS Long Term Plan for Cancer states that “where appropriate every person diagnosed with cancer will have access to personalised care, including needs assessment, a care plan and health and wellbeing information and support”.
We want all people with cancer across Wessex to experience culturally appropriate, integrated personalised care from the point of screening, throughout diagnosis and treatment, and after treatment has been completed or throughout palliative care. This includes support with healthy lifestyle choices and preventing, or best managing, consequences of treatment.
We want to make sure everyone has access to right intervention for them at the right time, in the right place and with the right people involved.
Our Personalised Care Projects
Our personalised care work programme includes a number of projects which will support the delivery of personalised care. The projects in the Personalised Care workstream include:
