Primary care is ever evolving to meet the increasing and more complex needs of local communities and populations. This will require new ways of working for the current workforce including providing high quality personalised care for people affected by Cancer.
The successful candidate will develop a service in collaboration with Woolston and Townhill Primary Care Network to prehabilitate and rehabilitate people with cancer or a history of cancer at any point in their cancer journey. This service will need to be regularly evaluated and flex and adapt to the PCN and patient population need as the service evolves.
Due to the complex nature of physical and psychological presentations you will need to demonstrate a wide range of knowledge, skills and attributes to ensure successful and appropriate management of symptoms of cancer and its treatments.
Experience of audit and service development will be beneficial as the successful candidate will need to collect data and evaluate the service alongside the project team. Additional responsibilities will include educating other colleagues in the PCN, influencing PCN cancer strategy, building relationships across sectors and driving improvements in cancer pathways.
The postholder will receive clinical supervision and mentorship within the PCN from the Cancer Clinical Lead and further education and support through the Southampton Federation. There will also be a substantial two week PCN induction to help with the transition to working in Primary Care and introduction to local systems, processes and governance within the PCN.
