Good working relations and well co-ordinated, effective MDT working is required for all teams looking after patients with cancer who have prehabilitation and rehabilitation. It is recommended that cancer MDTs should have representation from those delivering prehabilitation/rehabilitation therefore providing oversight of the needs of the person to ensure prehabilitation and/or rehabilitation takes place.
Community hub MDTs along with leisure-based facilities have an important role to play in supporting people with cancer with exercise, nutrition and psychological support. This will be in collaboration and part of the interdisciplinary approach to the persons care with specialist, hospital-based teams.
Good working relationships are needed to ensure:
- timely and appropriate referrals into services for screening, assessment, treatment or advice
- rapid access to appropriate professionals should there be a clinical concern e.g., metastatic spinal cord compression
- timely referrals from the service where concerns exist about progressive disease or complications or where further management or specialist care is needed e.g. referral to psychological support services
